Rates & Payment

• 50-minute individual therapy appointment = $200
  
• Payment is due at the time of each session, unless a different agreement is made.
  

Insurance Information

• I accept select insurance plans though Psychiatric Alternatives and Wellness Center group practice.  I am also an in-network provider for Lyra Health.

• If I am not an in-network provider on your plan and you wish to seek reimbursement from your insurance company, I can provide you with a monthly billing statement for reimbursement. These statements may also be submitted to obtain reimbursement of out-of-pocket expenses from your health savings account. Please keep in mind that insurance payments and reimbursements require a formal diagnosis and other information that will be disclosed to your insurance company and to any other parties they choose.
• If you check with your insurance company about whether your plan will reimburse you for my services, ask them these questions:
  • Does my health insurance plan include mental health services?
  • If my plan covers mental health services, how many sessions per year are covered?
  • Do a need prior approval or a referral from an in-network provider to see someone out of network?
  • How much will my insurance cover per session for me to see an out-of-network provider?
  • What is my deductible for the year, and has it been met?
  • How do I seek reimbursement for services provided by an out-of-network provider?

Cancellation Policy

• I expect that you will attend each scheduled session.
• If you need to cancel a scheduled appointment, you must do so at least 48 hours in advance of that appointment.
• Appointments canceled with 48-hour notice and missed appointments will be charged the full fee for the session.
